Development notes for a pediatrician visit

Bring a few dated examples of what your baby does, the context, and your questions. A short record can make a development visit more useful.

You do not need a perfect chart before asking about development. Write down two or three concrete moments, when they happened, and what you want to understand. Tell the clinic about a concern when booking so there is room to discuss it. The record helps a clinician hear your story; it cannot diagnose your child. CDC visit guidance

The short version to bring

  • One strength or new interest your baby has shown
  • A dated example of the movement, sound, play, or interaction you want to discuss
  • What happened just before and after, including who was there
  • Your one or two most important questions
  • Any existing health record or clinic form the team asked you to bring

AAP visit advice recommends keeping notes and completing any questionnaires sent before a specialist appointment. Observe, record, ask

A one-page visit note
